Division Commissioner Lieutenant General Khumalo failed to take action since February 2024 regarding General Madondo's presence in Crime Intelligence, which poses a security risk.
“The fact that no action was taken by Division Commissioner Lieutenant General Khumalo since 2024 February, raises serious concern as Madondo's mere presence in the Division Crime Intelligence poses a serious security risk to Crime Intelligence and the SAPS.”
Granting General Madondo access to Crime Intelligence increases the likelihood that State witnesses from that environment will be influenced or intimidated.
“most of the State witnesses would be from Crime Intelligence environment. Her access being granted to the same environment increases the likelihood of witnesses being influenced or intimidated.”
Mr Padayachee and General Matlou used reckless claims and accusations to impugn General Madondo's integrity in opposition to a bail condition amendment application.
“this resistance to an amendment of that condition has invited Mr Padayachee, General, what is their name, Matlou, to seriously impugn General Madondo's integrity and by throwing wanton claims against her and accusations.”
Mr Padayachee solicited an affidavit from General Matlou to resist General Madondo's court application to amend bail conditions.

General Matlou's affidavit filed in the bail appeal made irrelevant allegations regarding General Madondo's security vetting file, lack of security, and propensity for dishonesty without factual basis.

General Matlou colluded with Colonel Padayachee to oppose General Madondo's bail application despite having no involvement in the underlying legal matter.

Advocate Johnson signed and sent a letter dated 30 June 2025 to the National Commissioner stating that Generals Lushaba, Khumalo, Madondo, Lekalakala, Gabela, Ncube, and Brigadier Mokwele were prohibited by court order from entering any Crime Intelligence premises or contacting its personnel.
“On the 30th of June 2025, you penned, signed, and addressed a letter to the National Commissioner informing him that General Lushaba, General Khumalo, General Madondo, it is going to be projected here, General Lekalakala, and General Gabela, as well as Ncube, I apologise, I do not remember her rank, and Brigadier Mokwele. You state in this document... “Kindly note that none of the above named officers are allowed to enter any premises or office which forms part of the Division Crime Intelligence, or have any contact with its personnel as ordered by the Court.””
ADV Johnson says the recent J50 suspension related to vetting and appointment issues of General Madondo, not to appointments of Generals related to this matter.

File 9 contains information on aborted J-50 warrants for arrest of Generals Khumalo and Madondo and is still under investigation with no charge sheet available.
“It must be close by. And just for the record, that this is the file, the docket, we said it is still under investigation. It is fairly sensitive and it concerns the, I will call them the aborted J 50 warrants of arrest of Generals Khumalo and Madondo. I do not know if the request should go to you, but I have noted that in this file, there is no charge sheet.”
The affidavit that initiated the investigation and culminates in prosecution related to General Madondo's security clearance was commissioned by Mr Perumal and may contain questionable signatures possibly not belonging to Mr Adams.
“...the affidavit that initiates the investigation and culminates in the prosecution, correct? ... both the preamble statement and the affidavit, in respect of which it was suggested to Colonel Padayachee that there seems to be something wrong with the signatures there and that it looks like the signature that purports to be of Mr Adams may not be his. The affidavit is commissioned by Mr Perumal.”
General Madondo did not have a top secret clearance for many years and was refused such clearance while holding the rank of Brigadier, even during a promotional panel chaired by the deponent.
“... I was not aware, nor did anyone inform me, that Madondo did not have a top secret clearance, and was also refused a top secret clearance whilst she held the rank of Brigadier, including the time when I, that is this deponent, chaired a promotional panel, which resulted in her appointment as a Major General in the post, in that district... I was also unaware that her application for a top secret clearance was refused.”
Investigation into the lateral transfer of Major General Madondo from KZN to CI, assisted by General Khumalo and others, knowing she failed to secure a top secret clearance, is outside IDAC's mandate because IDAC does not investigate vetting processes.
“And assume all of that is correct. Assume all of that is correct. What does it have to do with IDAC? Is it IDAC's mandate to investigate lateral transfers within SAPS, and whether they were regularly done or not? ...But does IDAC investigate whether somebody's vetting is correct or not? Is that IDAC's mandate? No...”
The bundle referred to as FA 1 in the docket speaks to procurement of forklifts, but there seems to be a confusion since there is another FA 1 related to General Madondo's vetting.
“That speaks to procurement of forklifts . Correct . But your call the FA 1 we wanted was in relation to General Madondo 's vetting . So , there seems to be a different FA 1 . Each statement will have , naturally , its own annexure . So , that request stands .”
An allegation of fraudulent and corrupt conduct related to the questionable lateral transfer of Major General Madondo from KZN to Crime Intelligence Head Office by Lieutenant General Khumalo, assisted by other senior generals, who knew she failed top-secret clearance for many years.
“On about early October 2024 , I received information anonymously surrounding alleged fraud and corruption being perpetrated within the Division Crime Intelligence of the SAPS, the allegations related to the questionable lateral transfer of Major General Madondo from KZN to CI Head Office by Lieutenant General Khumalo, who was assisted by other senior Generalsto do so, knowing that she failed a top-secret clearance for many years.”
Only the vetting clearances of General Khumalo and General Madondo are questioned, not of other listed generals such as Madondo, Lekalakala, Van Vuuren, Bobby, and Ncube.
“In these, the vetting that has been questioned is that of General Khumalo and General Madondo. ... We account for only two ... whose vetting clearances are questioned.”
General Khumalo is facing charges and potential arrest under case 453/06/2025 related to vetting issues; however, the arrest warrant that was suspended relates to General Madondo's vetting issue and related fraud, not Khumalo's own vetting.
“We know that General Khumalo is facing charges and potential arrest under case 453/06/2025 and that is pertaining to the vetting issue. The warrant of arrest that was suspended relates to the vetting issue of General Madondo. Not General Khumalo's own vetting. It relates to a role he played allegedly in that contravention and the fraud related to that vetting.”
The warrant issued for General Madondo relates to alleged fraudulent documents made by her and not solely about the vetting process itself.
“It would only be the fraud relating to the alleged fraudulent documents, remarks, etcetera that would have been made by General Madondo.”
Allegations of fraud and corruption related to awarding tenders and contracts involve Generals Khumalo, Madondo, and Lushaba suspected of manipulating procurement processes according to Mr Adams.

Investigator Rapetsu and Advocate Ramsamy did not obtain any statement regarding General Madondo about the questionable appointment issues.

The summons document requests complete applications for security clearances and vetting files for various officers including General Khumalo, General Lushaba and General Madondo, despite documentation gaps in IDAC's records at the time.
“This summons effectively asks for complete applications for security clearances and vetting files for the officers listed therein, starting with General Khumalo, in respect of all the ranks he has held, starting with the Brigadier, Major General and Lieutenant General, and the pattern repeats itself. ... I think he is mentioned by name in the section 27 referral. So is General Lushaba and General Madondo. ... we have that problem of names that do not appear from documentation that IDAC has at the moment it takes this drastic step, appearing in, you know, in such documents as these summonses.”
General Madondo was laterally transferred from KZN to Crime Intelligence head office by Lt General Khumalo despite having failed to secure top clearance for many years.
“The allegations relate to the questionable lateral transfer of Major General Madondo from KZN to CI Head office by Lieutenant General Khumalo, who was assisted by other senior generals to do so, knowing she had failed to secure a top clearance for many years.”
There is no suggestion that Major General Madondo committed fraud by misrepresenting her clearance status or position.

IDAC is investigating fraud related to Major General Madondo’s lateral transfer, specifically allegations she committed fraud to obtain the transfer, but the charge is not mentioned in the discussed paragraph.

IDAC has a charge sheet listing fraud and corruption offences relating to the case of Major General Madondo and General Khumalo.

There is an ongoing fraud investigation into Major General Madondo related to her alleged fraudulent conduct to obtain a lateral transfer.
“There was in fact a fraud investigation that was undertaken, given her own fraudulent, allegedly fraudulent conduct.”
Advocate Johnson cannot disclose the details of the fraud Major General Madondo is said to have committed without casting aspersions on a matter pending review.
“I have the answer, but I am unable to answer it without casting aspersions on General Madondo on a matter that is still pending review.”
Mr Adams received information about General Madondo's security clearance application from documents pushed under his door in Parliament, which was not lawfully obtained.
“In his affidavit ... he says: “I attach hereto documents which were pushed under my office door at Parliament, and which relate to Madondo's application.””
The information about Madondo's security clearance was Crime Intelligence information that was not lawfully given to Mr Adams.
“It is Crime Intelligence information, he did not get it lawfully ... and he is telling your investigator ... that this was not given to me by General Madondo.”
General Madondo was given a lateral transfer without top secret clearance, posing a risk to Crime Intelligence (CI), South African Police Service (SAPS), and the country, according to an IDAC charge.
“We know the complaint is that she got this lateral transfer without a top clearance, top secret clearance certificate. At paragraph 3 it states: "Madondo poses a risk to CI, the SAPS and the country."”
General Madondo and Mr Adams do not have top secret clearance certificates but are handling intelligence documents they are not authorized to access, posing risks.
“General Madondo not having that clearance certificate poses a risk to CI, the SAPS and the country... equally too must Mr Adams. You agreed that he does not have a top secret clearance certificate, and yet he is handling documents seemingly that come from Crime Intelligence and for which he does not have lawful access or authorised access.”
General Madondo was charged by IDAC as part of an investigation into unauthorized possession of intelligence documents.
“I D A C charged General Madondo in terms of CAS 453/06/2025...”
Mr Adams is also being charged in the same investigation concerning unauthorized access to intelligence documents as General Madondo.
“If General Madondo not having that clearance certificate poses a risk... equally too must Mr Adams... handling documents seemingly that come from Crime Intelligence and for which he does not have lawful access...”
General Madondo was arrested in connection to this contravention but not by IDAC directly; IDAC sought his arrest.
“He is arrested I think amongst others for this contravention... Not by IDAC... It is IDAC who sought to arrest General Madondo.”
IDAC possesses documents marked FA 1 related to Madondo's application for security clearance, which Mr Adams says were shoved under his door at Parliament.
“Mr Adams says... those which relate to Madondo's application for security clearance marked FA 1. That tells me that IDAC has those documents. They are within IDAC.”
There were suspicions that General Khumalo, Madondo, Madondo, and Lushaba manipulated procurement processes and processes to appoint individuals, but evidence shows approvals and signatures were made by relevant officials including General Lushaba as CFO.
“I suspect that General Khumalo and some other officers in Crime Intelligence, including Madondo and Lushaba, were manipulating procurement processes and processes to appoint”
General Lushaba, as CFO, signed off on the procurement approvals and recommendations for the forklift acquisition, and it is unclear why Generals Khumalo and Madondo are implicated in the forklift acquisition issue.
“at the bottom of 3510, it is signed by Major General CFO ... General Lushaba's signature as CFO ... I do not see how General Khumalo and General Madondo are pulled into this forklift acquisition issue”
Investigations into General Khumalo and others suspected of procurement manipulation are ongoing.

Ms Mokwele did not fail to apply for or get top secret clearance where required. The matter of failing to apply for top secret clearances is under review in relation to General Madondo.

Several highly skilled and qualified senior police officers were overlooked in favor of Mrs Mokwele for the position, with involvement from generals Khumalo CI Head, Lushaba CI, Madondo CI, and Ligagala CI.
“there were several highly skilled and qualified senior police officers who applied for the post , but were overlooked by the CI management , who favoured Mrs Mokwele over them . The generals involved are Khumalo CI Head , Lushaba , CI , Madondo , CI , Ligagala CI , amongst others .”
Several highly skilled and qualified senior police officers who applied for the post were overlooked by CI management in favour of Mrs Mokwele.

The National Director of Public Prosecutions (NDPP) has the authority to review prosecutorial decisions on matters such as the Lushaba and Mokwele cases, and cases with warrants of arrest for Generals Khumalo and Madondo.
“The NDPP has the right to review, or have to be reviewed the Lushaba matter, which is in court, the Mokwele matter, which is in court, and the matter for which warrants of arrest had already been obtained for Generals Khumalo and Madondo”
The section 27 affidavits specifically identify General Khumalo, General Madondo and General Lushaba, but do not reference persons identified under items 4 to 12 in the annexure.
“I cannot find in the section 27 the persons identified under items 4 to 12 ... It does not reference any of the people listed from 4 till 12.”
The referral affidavit raises concerns about General Khumalo, General Lushaba, and General Madondo manipulating procurement processes to appoint and promote certain members.

There are multiple separate investigations related to the Section 27 affidavit referral, including the Lushaba matter, the Mokwele matter, and a third investigation involving Lieutenant-General Khumalo and General Madondo.
“There is then a third investigation. That is the one for which the warrants were held over, just for context, in terms of Lieutenant-General Khumalo and General Madondo. And then there are other matters that also emanate from this complaint.”
The same affidavit alleges that persons appointed to strategic positions are suspected to be loyal and close to Khumalo, Lushaba, and Madondo or those close to them.
“It speaks of persons being appointed to strategic positions and that such persons are suspected to be loyal and close to Khumalo, Lushaba, and Madondo and those close to them.”
There are at least six separate investigations relating to a Section 27 referral involving General Lushemba matter, Mokwele matter, and others including Lieutenant-General Khumalo and General Madondo.
“When the prosecutors got the Section 27 referral, Counsel, they broke it up as follows. Started with the General Lushabama matter. Yes. That is the first matter. The Mokwele matter being ... there is a third investigation ... warrants were held over for Lieutenant-General Khumalo and General Madondo ... there are other matters ... five, six ... there would be six.”
Mr Adams has a growing concern that General Khumalo, General Lushaba, General Madondo, and other senior Crime Intelligence officers are involved in systematic capture of the Crime Intelligence division.
“it expresses his growing concern that the Head of Crime Intelligence, General Khumalo, General Lushaba, General Madondo, and other senior Crime Intelligence officers are busy with systematic capture of Crime Intelligence division”
General Khumalo, General Lushaba, General Madondo and appointees are suspected of siphoning money from secret fund accounts across about 10 secret fund points in the country, which need investigation.
“My suspicion is that General Khumalo, Lushaba, Madondo and many of those who they have appointed are actively syphoning money from the secret fund accounts in the country. I understand that there are about 10 secret fund points in the country where monies can be drawn from and all of these extraction points need to be investigated.”
The section 27 affidavits specifically identify General Khumalo, General Madondo and General Lushaba, but do not include the persons identified under items 4 to 12 in the annexure presented by Advocate Johnson.

Mr Adams has growing concern that senior Crime Intelligence officers, including General Khumalo, General Lushaba, and General Madondo, are involved in systematic capture of the Crime Intelligence division.
“It expresses his growing concern that the Head of Crime Intelligence, General Khumalo, General Lushaba, General Madondo, and other senior Crime Intelligence officers are busy with systematic capture of Crime Intelligence division.”
Mr Adams suspects that Generals Khumalo, Lushaba, Madondo, and many they have appointed are syphoning money from secret fund accounts across approximately 10 secret fund points.
“My suspicion is that General Khumalo, Lushaba, Madondo and many of those who they have appointed are actively syphoning money from these secret fund accounts in the country. There are concerns regarding General Khumalo, General Lushaba, and General Madondo manipulating procurement processes and appointing/promoting certain members loyal to them in Crime Intelligence.

A section 28 summons dated 6 February 2025 required original complete applications for security clearances and vetting files for senior officers of the South African Police Service including Generals Khumalo, Lushaba, Madondo, Ncube, and others.

A report by Major-General Acting Divisional Commissioner Inspectorate NMD Ladla dated 4 November 2024 outlines the opening of two sets of dockets and discusses criminal allegations against Generals Khumalo, Lushaba, Madondo and Brigadier Ncube including abuse of secret service accounts, cover-up of criminal incidents, tax evasion, and nepotism.
“The criminal allegations in the six cases which were opened are levelled against Generals Khumalo, Lushaba, Madondo and Brigadier Ncube and are centred on four specific matters, namely: Abuse of Secret Service Account ... Cover-up of criminal incident linked to Villieria CAS number ... Failure to address Major-General Madondo's results of the polygraph examination ... Practices of nepotism”
Major-General Madondo allegedly failed a polygraph examination in July 2024, and Brigadier Ncube was tasked to ensure the matter would not be addressed or investigated.
“The complainant alleges that Major-General Madondo failed her polygraph examination in July 2024, whereafter the Head of Vetting, Brigadier Ncube, was asked to ensure that the matter would not be addressed or investigated.”
There is an investigation into manipulation of security services vetting processes resulting in unlawful issuing of security clearance for certain individuals including Khumalo, Lushaba, Madondo, Ncube, and Dobie.
“The investigation pertains to manipulation of security services vetting processes which resulted in the unlawful issuing of security clearance.”
There is no complaint or allegation in the Crime Intelligence file or any other document that Major General Madondo is involved in a scheme to capture Crime Intelligence.

The summons issued on 6 February 2025 signed by Advocate Johnson requests original personnel files for senior officers of SAPS including Khumalo, Lushaba, Madondo, Moyana, Ncube, Van Vuuren, Mokwele, and Bobi, regarding promotions and appointments.
“This summons requires all original files , records , correspondence , and documents regarding the promotions , appointments of the following senior officers , Khumalo , Lushaba , Madondo , Ncube , Van Vuuren , Mokwele , Bobi .”
The appointments of the persons listed including Khumalo, Lushaba, Madondo, Moyana, Ncube, Van Vuuren, Mokwele, and Bobi are under investigation, specifically their promotions.
“During the briefing by Chief Investigator Perumal , the appointments of these people mentioned here , right , and this is what we were requested to subpoena because the appointments are also under investigation .”
There is a complaint about appointments of Lushaba, Madondo, Moyana, Ncube, Van Vuuren, Mokwele, and Bobi, which is the subject of a lawful investigation by IDAC.
“The same with Lushaba, Madondo, Moyana, Ncube... Van Vuuren, Mokwele and Bobi. There is such a complaint.”
General Madondo and other people are also involved in other docket cases, so the Chief handles those, while the witness's specific matter concerns a request for Mokwele and Lushaba.

The ongoing broad investigation into Crime Intelligence includes the appointments and promotions of General Khumalo, General Madondo, and others, integrated under CIN 130/02/12/2024 inquiry number opened in December 2024.
“We now have come to understand that you are investigating specifically the Mokwele appointment, but that happens in the context of a broader investigation... with many legs. So we are trying to find documents pertaining to the investigations of the appointments and promotions of General Khumalo, General Madondo, all those people listed there... that investigation will be part of that CIN number 130/02/12/2024”
Khumo, Madondo, Lekalakala, and Gabela unduly favoured Mokwele above experienced Colonels within the immediate environment who met all the requirements during the panel sittings.

Generals Khumalo, Madondo, and Lekalakala were granted bail with the condition prohibiting them from being at any Crime Intelligence premises while their criminal trial is pending.

The affidavit signed by Colonel Padayachee on 24 June 2025 includes a draft chargesheet alleging corruption, defeating or obstructing the administration of justice, and perjury against General Khumalo and others.
“The attached draft chargesheet outlines the acts committed by each of the seven suspects in a very comprehensive way, which includes charges of corruption, defeating or obstructing the administration of justice and perjury.”
The second matter involved an arrest of several senior Crime Intelligence members: Generals Khumalo, Lekalakala, Lushaba, Madondo, Brigadiers Mokwele and Ncube.
“The second matter was General Khumalo, General Lekalakala, General Lushaba, General Madondo, Brigadier Mokwele, and Brigadier Ncube.”
The speaker claims to have been a mentor to General Madondo and had long working relationships with General Lekalakala since the 2000s.
“I was General Madondo's mentor. I had known General Madondo from the 000s . ... General Lekalakala , we have a long working relationship since the 2000s .”
General Khumalo, Generals Lushaba and Madondo and other senior Crime Intelligence officers are suspected of systematically capturing the Crime Intelligence division.
“General Khumalo and the other persons listed there, including Generals Lushaba and Madondo and other senior Crime Intelligence officers, are busy with the systematic capture of Crime Intelligence division.”
General Khumalo and other senior officers in Crime Intelligence, including Madondo and Lushaba, were manipulating procurement processes to appoint and promote certain members to higher strategic positions within Crime Intelligence.
“I suspect that General Khumalo and some of the other senior officers in Crime Intelligence, including Madondo and Lushaba, were manipulating procurement processes and processes to appoint and promote certain members to higher strategic positions within Crime Intelligence.”
General Khumalo, Lushaba, Madondo and others appointed members who were actively siphoning money from secret fund accounts in the country.
“General Khumalo, Lushaba, and Madondo and many of those who they have appointed are actively siphoning money from these secret fund accounts in the country.”
There is a suspicion that General Khumalo, Lushaba, Madondo, and many appointed persons are siphoning money from the secret service fund in the country.
“My suspicion is that General Khumalo, Lushaba, Madondo, and many of those who have been appointed are actively siphoning money from the secret service fund in the country.”
Documents did not specifically show or point to General Khumalo or Madondo's involvement in siphoning funds but indicated there were possibilities of money being siphoned unlawfully from the secret service fund.
“The documents did not show and point to Khumalo, Madondo or what. The documents showed that there were possibilities of money being utilized unlawfully from the secret service fund, money being siphoned.”
Sergeant Nkosi shared names and case numbers of people arrested, including General Khabela, Brigadier Madondo, and General Khumalo, but Deputy Chief Dhlamini did not know who these people were or why the diary was shared with him.
“This is not just a sharing of what is on social media. He gives you names of people that have been arrested ... General Khabela... Brigadier Madondo... General Khumalo... I do not know who Kabela is... I knew who Madondo is... I knew who Khumalo is... You have just said you know nothing about this.”
