M R O 'S U L L I V A N states that in 2013, a person shifted blame for a loan involving 408,000 Rand onto Cyril Beeka, who was murdered in May 2010, implying it is convenient since the deceased cannot respond.

On 9 January 2014, Radovan Krejčer was arrested and at the same time seven people were arrested planning to murder M R O 'S U L L I V A N.
“On January 9 , 2014 ... Radovan Krejčer was arrested ... and then on the 9th January in 2014 , seven people were arrested just down the road from my house , planning to murder me”
Colonel Steyn ultimately left the police force and the matter was not pursued further by M R O 'S U L L I V A N due to limited resources and bigger issues demanding attention.

Radovan Krejčer was in prison at the time M R O 'SULLIVAN was threatened with murder because he had the witness's cell phone records.

Gratification as misconduct pertains to receiving a loan or benefit in exchange for favours, described as a general corrupt relationship rather than a quid pro quo.

Mr O'Sullivan alleges that a person was parachuted into a position, bypassing all procedures in the recruitment process without regard to cost.
“The information I received was that she was parachuted in, which is an expression meaning bypass all the procedures, appoint her at any cost.”
Mr O'Sullivan received a tip-off implicating a woman recruited without going through proper recruitment procedures and made a complaint to the OIGI about it.
“My complaint was that she had been recruited without going through proper recruitment procedures. The informant that came to us told us that this person had been recruited to a position which other people had applied for and were competent to do, but that they had bypassed the procedures.”
Mr O'Sullivan denies being the source of claims bandied about in public that the woman was appointed as a tea lady at BMW.
“You are not on record as having suggested that she was a tea lady at BMW... Absolutely not... Definitely I am not the source of such activities.”
Mr O'Sullivan stated that the expertise required for the position was mechatronics, which the woman did not discuss with him when he inquired about her qualifications.
“The expertise that was required here is mechatronics, mechatronics... When I asked her about her qualification, she did not want to discuss it with me.”
Mr O'Sullivan conducted background checks and established from BMW Motors that the woman was an admin clerk, not holding the required qualifications, and she was uncooperative when questioned about her qualifications.
“I spoke to BMW Motors and I was able to establish what she was doing there and that comprised the report which I gave to the OIGI... It is an admin clerk... And when I asked her about her qualification, she did not want to discuss it with me.”
Mr O'Sullivan denies slipping the investigation dossier under a certain member of Parliament's door and denies that anyone got any dossier from him.
“I am not in the business of jumping on a plane and flying to Cape Town to slide a dossier under anybody's door... I do not send any dossiers... Absolutely did not get any dossier from me.”
Mr O'Sullivan clarified that his complaint was not about a criminal offence but a complaint to the OIGI related to improper recruitment procedures and broader complaints involving property purchases.
“My complaint was not a criminal complaint. It was a complaint to the OIGI that, and, by the way, it's at with other aspects of a broader complaint involving properties that were being purchased and other issues.”
