Warrant Officer Mpangase did not put exhibits back into the SAP13 register after booking them out, instead they were transported to and stored at Port Shepstone DPCI vault.
“we had already recorded them in SAP13 in Isipingo. And then at that time they were being transported to Port Shepstone”
After booking out exhibits from SAP13 register, it is required by national instruction to account for them properly and enter them into the register again if returned.

Maj-Gen Senona confirms familiarity with the requirement to record exhibits in SAP13 where the crime was committed and then transfer them to the relevant unit's SAP13 register.
“You are familiar with that ... Correct. Yes, Commissioner.”
During the storage of the drugs in Maj-Gen Senona's presence, there was no compliance with entering them into SAP13 as required by paragraph 12(2).

Brigadier Nyuswa confirms he only learned at Port Shepstone that the exhibits were not sealed in exhibit bags and instructed Mpangase to ensure exhibits were put into exhibit bags and registered into SAP13.
“You only get to know when you are there that they did not put it in the sealed bags. That is why you then give Mpangase that instruction to do it.”
WITNESS I was requested to bring the SAP13 book from their office to the Pretoria West Training College as part of the 2021 cocaine seizure operation.
“I was requested to go to office, obtain SAP13 page 11 of 135 book from my office, and bring it to Pretoria West Training College, where the seized Cocaine was booked at for safety.”
Assistant Director Maphosho indicates that there are two different forensic bag numbers SAP13 and another one submitted to the lab and that this discrepancy was not directly questioned to Captain Nortje.

According to Colonel Jacob, Maydon Wharf police station has its own exhibits to attend to, similar to Isipingo police station; securing appropriate storage space is difficult because stations do not want to give up SAP13 exhibit room space.
“Maydon Wharf has their own exhibits to see , to attend to ... those stations know that the lab takes a long while to house those exhibits ... but no station wants to give away the SAP13 space”
