Humbulani Innocent Khuba

IPID formed a task team to manage Mr O'Sullivan's complaint against General Phahlane, headed by Humbulani Innocent Khuba, now Limpopo Director of IPID.

“IPID formed a small task team, and my understanding is that the task team was headed up by this gentleman Humbulani Innocent Khuba. And he is currently the Limpopo Director of IPID... he was brought to Pretoria because they wanted somebody from outside the province to case manage my complaint against General Phahlane”

According to Humbulani Khuba, unlawful conduct of General Mkhwanazi was present during the investigation period.

Humbulani Khuba's affidavit is missing original statement submitted to Zondo Commission but draft affidavits and email correspondence exist showing a process of iterative drafting.

“I cannot find the original affidavit that I submitted in 2020 to the Zondo Commission. However, what I have been able to locate are the emails from the Zondo Commission giving me draft affidavit... making affidavits for a commission, it is an iterative process”

In February 2018, Humbulani Khuba met Major General Nemutanzhela of Crime Intelligence in Pretoria for a meeting involving allegations of interference.

“in February 2018 he got a phone call from Major General Nemutanzhela of Crime Intelligence... I remain in the lobby while we waited for the anonymous witness... after one of his calls Nemutanzhela said to me that we could go now... I saw Lieutenant General Mkhwanazi... Mkhwanazi asked us to switch off our cellphones and hand them to him... Present in the meeting was General Mkhwanazi, Nemutanzhela and me”
